Editorial Branding Styled Shoot- Makeup Release

Katie Carodine Photography is the official organizer and planner of this styled shoot and her agreement with you is as follows:

For the Editorial Branding styled shoot on the day of August 17th at Starlook Studio located in McMinnville Oregon:

1. Katie Carodine will coordinate with all of your clients for that day. You must supply all necessary and required equipment to provide the services, and you will be paid $35 per service.

2. You must arrive on location for the scheduled time that Katie Carodine provides you. You are required to be reviewing the planner and schedule for the day.

3. Katie Carodine Photography will supply model release forms for each model with permission for you to use their images.

4. As an attendant at our styled shoot, you recognize that there is possibility of lack of coordination or clumsiness resulting in bodily injury or harm. As an attendee you agree to not hold any attending vendors or models accountable for injuries you may incur at this event. You also agree to provide the safest environment possible for each person attending by not willfully or intentionally causing harm to others. This way we can all know we are on the same page with creating a fun and enjoyable experience for everyone attending. If anyone is found purposefully harming other attendees, they will be immediately removed from the premises and will not be allowed to attend future events.

5. As the event planner, Katherine Carodine will also be taking photographs at the event. You understand that as an attendee, you will be in these photos and agree to sign a model release for using the images. No photos of you will be sold and will strictly be used for advertising and marketing purpose

6. As an attendant of the event, you also agree to do your part to not destroy or damage any of the sets or props that are provided. If you are found in part responsible for property damage, you agree to help with replacement costs or cleaning fees.

I have read the above authorization, release, and agreement, prior to its execution, and I am fully familiar with the contents of this document. This document shall be binding upon me and my heirs, legal representatives, and assigns.
